HOOKE Jeremy Peter (Jay) Passed away peacefully at home with family by his side on 16th December 2019, aged 53 years. A much loved husband, brother and son who will be dearly missed. Funeral service to be held at North Wiltshire Crematorium on Friday 3rd January 2020 at 3:00pm. Family flowers only but donations to Brighter Futures - Dove Ward, Brighter Futures - Osprey Day Centre Unit, John Muir Trust and Prospect Hospice would be gratefully appreciated c/o Hillier Funeral Service, 170 Victoria Road, Swindon SN1 3DF.
